Are the drops FDA Approved?

Semaglutide and Tirzepatide drops, while gaining popularity in the weight loss market, are not currently approved by the U.S. Food and Drug Administration (FDA) in their drop formulations. Although both medications are FDA-approved for specific injectable treatments related to diabetes and, in some cases, chronic weight management, their use as compounded oral drops has not undergone the rigorous testing and regulatory review required for FDA approval. 


 While compounded oral versions of semaglutide and tirzepatide are not FDA-approved, there is ongoing research into their potential. Novo Nordisk has submitted a marketing application for an oral version of semaglutide, which, if approved, would become the first oral GLP-1 therapy for chronic weight management in the U.S. The FDA has accepted this application, and a decision is expected later in 2025 . Similarly, Eli Lilly is developing an oral GLP-1 medication, orforglipron, which has shown promising results in clinical trials . These developments indicate a shift towards more convenient oral formulations, but until FDA approval is granted, compounded versions of these drugs remain unapproved and carry potential risks.

Sublingual Versus Injectable

Effectiveness of Oral Tirzepatide/Semaglutide (Sublingual Drops)
While oral (sublingual) Tirzepatide/Semaglutide offers a needle-free alternative, it is important to understand that it is significantly less bioavailable than the injectable form. Although the same peptides are used, the body absorbs them differently. Based on current compounding methods and available data:

  • Injectable bioavailability: ~89%
     
  • Sublingual bioavailability (with submagna base): ~10%
     

Due to this lower absorption, sublingual Tirzepatide/Semaglutide must be taken daily and often at higher doses to achieve comparable effects to the once-weekly injections. As this formulation is still relatively new, results may vary, and clinical feedback is still evolving.

Semaglutide – Side Effects & Contraindications

  • Pancreatitis (inflammation of the pancreas)
  • Vision changes (especially in those with diabetes)
  • Hypoglycemia (especially when combined with other diabetes medications)
  • Kidney problems due to dehydration from vomiting/diarrhea
     

Semaglutide is not suitable for:

  • Individuals with a personal or family history of medullary thyroid carcinoma (MTC)
  • Those with Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
  • Patients with severe gastrointestinal disorders, such as gastroparesis
  • People under 18 years old, as safety is not established in this group

Tirzepatide – Side Effects & Contraindications
